The problem happens rarely. I cannot reproduce it.

I had multiple tabs open, each connected to an SSH session on other machines / VMs. I clicked on a tab, refreshed my local directory, click-and-dragged a file from local-to-remote to transfer it, then clicked off the window. I did not see the precise moment the error occurred.
